WHAT THE SERVICE COVERS

The minimum useful intake record.

Collect observable facts, operating impact, access requirements, and the definition of completion before dispatch.

Site identity

Site name, complete address, location number, onsite contact, operating hours, and check-in instructions.

Observed condition

Exact location, what staff observed, when it began, whether it is active or intermittent, and safe-to-collect photos or video.

Operating impact

People or property at risk, closure status, affected utilities or access, worsening conditions, and the needed response window.

Access and constraints

Parking, loading, security, restricted areas, business-hour limitations, and any site-specific requirements.

Approval path

Not-to-exceed amount when applicable, additional-work approver, estimate requirements, purchase order, portal, IVR, and invoice rules.

Completion definition

Restored service, documented stabilization, repair recommendation, estimate, photos, notes, or a scheduled follow-up.

WHEN TO ESCALATE

Use the human emergency path when risk is present.

A work-order record does not replace emergency procedures or qualified judgment. Escalate immediately when people, property, essential operations, or site security may be at risk.
